Crafting Occult Gems requires a Fugitive Head, along with 1,000 Restless Rot and 50,000 Gem Fragments, making Fugitive Heads a crucial component.

Acquiring Fugitive Heads involves completing Whisper bounties in Headhunt zones—new areas introduced in Season 7. Successfully completing these bounties presents a chance for an Uprooted Cocoon to appear. Defeating the Headrotten Boss emerging from the Cocoon may yield a Fugitive Head.

This process is largely chance-based. To increase your odds, use the Draught of Whispers Elixir (obtained from Exposed Roots, Whispers Cache, and Coven’s Cache). This elixir boosts the Fugitive Head spawn rate by 50% and increases the likelihood of Forgotten Altars appearing.

Finally, difficulty matters. Fugitive Heads have an extremely low spawn rate below Torment difficulty. While challenging, tackling Torment difficulty increases your chances significantly. Remember, Season 7 runs until April, giving ample time to collect Fugitive Heads.
