Mikami a déclaré son désir d'une suite, qualifiant l'original de ses favoris personnels. Suda51, la force créatrice derrière Killer7, a répondu avec son propre enthousiasme, suggérant des titres de suite potentiels comme "Killer11" ou "Killer7: Beyond".

Malgré son caractère culte, une suite est restée insaisissable. Même après une remasterisation sur PC en 2018, Suda51 a exprimé son intérêt pour une « édition complète », une idée que Mikami a rejetée de manière ludique.
